To me it's a problem of perspective. Most people working in the transportation department do not have the perspective of a cyclist or pedestrian - they perceive the environment only from behind the wheel of a car.
Until we start looking at issues from multiple perspectives, infrastructure that doesn't serve the needs of pedestrians and cyclists will continue to be the norm.
